The IRS has revised Form 8915-F, “Qualified Disaster Retirement Plan Distributions and Repayments.” The form is used to report on distributions made in response to qualified disasters.
The revised version, and its instructions, are early releases and are in draft form. The IRS released them in order to provide information only—the draft form is not to be used for filing, nor are the instructions in final form to be used in completing the form for filing.
Form 8915-F is a redesigned version of Form 8915.
Form 8915-F is to be used for distributions for qualified 2020 disasters, as well as qualified 2021 and later disasters) and for each year of reporting of income and repayments of those distributions. It also is to be used after 2020 instead of Form 8915-E for coronavirus-related and other 2020 disasters.
The boxes checked in items A and B on the Form 8915-F help the IRS determine the exact year of the form being filed, as well as the year of the qualified disasters, qualified disaster distributions, and qualified distributions being reporting.
Qualified 2021 and later disaster distributions (also known as qualified disaster recovery distributions).
Under the provisions of Section 331 of the SECURE 2.0 Act of 2022, Form 8915-F may be used if: (1) one was adversely affected by a qualified 2021 or later disaster and (2) received a distribution described in Qualified Disaster Distribution Requirements or Qualified Distribution Requirements, in the instructions for the form.
For qualified 2020 disasters, the list of qualified disaster areas appears in Appendix B in the Instructions for Form 8915-F (revised in February 2022).
For qualified 2021 and later disasters, the qualified disaster area is the state, territory, or tribal government in which a qualified disaster occurred.
A qualified disaster is a disaster that the President has declared to be a major disaster. It is also known as a federally declared major disaster. The Federal Emergency Management Administration (FEMA) numbers for these major disasters include the letters "DR" in capital letters.
